Unfortunately a common practice. Blame for failure should be aimed at company senior leadership. Failure to communicate what is expected of managers and give them the resources and processes to meet company expectations will inevitably result in failure.

ha ha ha ha this used to be a line in a famous film or prgram when i was a kid but i cant think of it,

"what we have here, is a failure to comunicate"

think it may have been a yank film cant remember, any ideas??
