If you're flying into Bangkok the smoothest way would be to connect onto a Bangkok Airways flight. They have an airport in Trat and the ticket includes bus and boat transport to Koh Chang.
Probably not the cheapest way though.

Great pics AG. I don't really like beaches , sea and sand much but looking at your photos almost made me want to go and see for myself. Very enticing.
