Evidence you have sufficient funds .
Application form .
You can apply to any country you like . Technically you should be applying for France but its a Schengen Visa and covers everywhere . You need to book an appointment to go in and have fingerprints taken ( and can wait 2 months before getting an appointment ) . Then come back 3 days later for the Passport .
We have acquired a visa every time but it's a MASSIVE pain in the backside.

Tell her to not bathe for a month, cover herself in copious amounts of cheap perfume, chew a few garlic cloves and sing the Marseillaise when she gets to the border. They'll think she's a local.
Is there a need for her in France to warrant all the bother ?

Most of the time we went to the Belgian Embassy .
It's the same visa but not so many people go to Belgium .
So we never had the queues that you see at the French or the Italian or Spanish Consulates.
