You mean like a squat toilet surrounded by a bamboo wall in the jungle? Just put lots of flowering plants in there. A b*tch when it rains tho.
I was at a resort in the Mae Kampong area outside of Chiang Mai and one of the chalets there had a bathroom that wasn't so much outside but it had a transparen roof on it which made the whole thing nice and bright. (Some trees etc provide shade so it didn't turn into an oven). The bathroom itself featured a lot of river stones set in cement, as well as wood.
Question to the OP, do you mean something that's separate from the main house, or just something that's semi open on some sides?

Pattaya is an interesting toilet to visit.

I used to have an outdoor shatter in (or should I say outside) my old place.
Left hand door contained run of the mill squatter. I'd only enter that one when the situation was critical.
Right hand door was home to a brand spanking new piece of porcelain and a shower.
Please note. One should consider sound proofing such a convenience. I'd occaisionally here the neighbours sniggering when the bowel movement was extra fruity.

WLL, your description of Mae Kompong was what i had in mind. I was thinking more attached to the house than an actual outhouse. A waterfall shower with a lot of stones and wood.
It's a bitch to clean though compared to tiles.But if you have a maid then that's her issue. Anyway I've been pondering this issue myself: Where to put the bathroom in a Thai style wooden structure in such a way that it doesn't end up as a brick & cement eyesore disrupting the overall look of the wooden structure. Having it outside is one option, having it in the middle under the house (instead of on a side) is another, or even on the first floor on the side but somehow avoiding bricks and tiles.
There are practical concerns though; outdoors in lush greenery sure sounds nice, but the reality of it all would likely be dodging snakes, centipedes and mosquitos at 4am when going for a pee.
So I'm leaning towards having it on the upper level, but avoiding bricks & tiles as much as possible, and have it as open as possible while probably still under the/a roof structure of the house

We have an outside dunny on the farm.
We are building a dunny that will have no roof and truly outside, if it rains you get wet, if there is lightning you get struck, if there is thunder then thats just you.
