An artillery shot from HTMS Chonburi accidentally struck HTMS Khirirat on Thursday, causing a fire and injuring three people. Both Thai navy vessels were anchored at Laem Thian Pier, Sattahip Naval Base, Chonburi province.

The incident took place at 11.50am when the naval artillery shot discharged from the front artillery of HTMS Chonburi hit the rear section of HTMS Khirirat.

HTMS Chonburi had returned from a firing drill on Wednesday and had returned to the base for repairs, as one of its artilleries had malfunctioned during the operation, with an unexpended round still in its ported barrel.

Launching a round usually involves several steps so this is a pretty wild story. Looks like they had to figure out some way to save face knowing how screwed up this is:

The 1982 Harpoon missile misfire incident, in Danish dubbed hovsa-missilet ('The Whoops Missile'),^[1] was an unintentional discharge of a live Harpoon missile by Danish frigate HDMS Peder Skram during a training maneuver in the Kattegat on 6 September 1982.^[2] The missile traveled 34 kilometers at low altitude, severing several power lines before eventually striking a group of trees and exploding. The fireball and subsequent shockwave destroyed four nearby unoccupied summer cottages and caused minor damage to a further 130 buildings in the area. There were no injuries.
Cause[edit]

A navy investigation into the matter concluded that a technical malfunction was the cause of the launch as it happened without the launch key being activated. The missile system had undergone maintenance and was in the process of being checked by an expert from the Navy Material Command (abolished in 1985, prior to the Packard Commission^[3]), who was later charged and convicted for negligence, though most charges were later dropped following a second investigation. McDonnell Douglas, the manufacturer of the missile later paid the Danish government compensation, which covered all the damage made by the missile.^[4] It was applied a termination clause like the ones which in 1999 became a recommended standard for contracts valued at less than $100K.^[5]

The Armed Forces and the American arms giant, which had built the missile system, agreed. They pointed to the same man.

A man who unsuspectingly put the safety of the Danes at risk when he entered the fateful numbers on the missile console: 1-2-3.

The Navy has relocated HTMS Chonburi out to sea, from the naval base in Chon Buri province, after a jammed artillery shell was accidentally discharged into HTMS Khirirat, which was docked nearby.

The accident caused a fire on HTMS Khirirat and injured 14 sailors.

The navy’s commander-in-chief, Admiral Adoong Pan-iam, explained that the relocation of HTMS Chonburi is a preventive measure, to avoid a repeat of the accident, adding “The ship will be thoroughly checked for any more jammed artillery shells or other faults.”

He was responding to widespread doubts about the reason for the relocation, as the frigate is under investigation following the accident. An investigation team was established yesterday to determine the cause of the accident and report to Admiral Adoong within three days.

Adoong also said that eight out of the 14 officers injured in the accident have already been discharged from hospital. Most of them were injured by inhaling hot air and smoke during the fire.

HTMS Khirirat, built at the Norfolk dockyard in the United States in 1972, entered service with the Thai navy in 1974. It has a displacement of 1,172 tonnes, is 82.5 meters long, and 9.9 meters wide and is equipped with one 76-mm cannon, 20mm and 12.7mm guns and a depth charge launcher.

The fully armed Chonburi class patrol craft was built by the Italian shipyard, Cantiere Navale Breda, and is designed to be fast and manoeuvrable, with a maximum speed of 29 knots and a range of up to 2,500 miles. It has been in service with the Thai navy for 41 years.

Commander of HTMS Chonburi transferred pending investigation

The Navy has transferred the commander of HTMS Chonburi to the Royal Thai Fleet Command on a temporary basis, pending the outcome of the investigation into an accident in which his ship accidentally fired at HTMS Khirirat on March 14.

The accident caused a fire on HTMS Khirirat and injured 14 navy officers, mostly due to hot smoke inhalation. The accidental firing also damaged four compartments on the ship.

The Navy has set up an investigation panel to look into the cause of the accident. Both ships were docked next to each other when the accident happened.

Rear Admiral Chalermchai Suankaew, commander of the Patrol Squadron, said in a press conference yesterday that HTMS Chonburi conducted a firing drill at sea on March 13, but experienced two artillery shell jams, which they managed to repair.

The problem happened a third time, which prompted the commander to return the vessel to Laemtien naval base for repairs. It is reported that, during the repairs, the artillery barrel was set at a 45-degree angle for safety. The barrel was later lowered by five degrees. It was then that the accident occurred.

The problem is obvious. No need for an investigation. A ship long past it's use by date with no money to properly maintain all it's equipment will cause things like this to happen.
