Yeah, that puzzles me too. Extreme left means things like nationalizing private industries, compulsory unionization, 90% income tax on high earners etc. I've never heard anything that radical from Obama - he's just the usual luke-warm left of center boring democrat. How people can stand to hear him parrot on about 'change' when his policies are about anything but change is a mystery.

re health care: something needs to be done about the costs. a retiree today can expect to spend 1/3 of their life time earnings to get health care.
