They don't specify. Are you a US citizen? When I applied for my visa (Houston consulate by mail) I gave them evidence of my gross annuity. They accepted that.
When I have applied for extensions I have gone to the US Embassy to get a notarized statement about my income. I always use the gross amount. But, the US Embassy doesn't require proof, so you can put whatever you like. Thai Immigration accepts whatever is down on the form that is notarized by the embassy.
