I've recently had my work permit renewed on Phuket. 3rd year now, same company, various positions within it. No health cert needed other than the initial one on 2010. This year curiously HR needed a photo of me at work.... (i.e. sat behind a desk) for the Labour Dept.
Anyway all clear until February 2014 now.
It would have been amusing if they had asked for blood tests this time around. I had been in hospital for 7 weeks and have blood test results for all imaginable diseases in a file about an inch thick!
UPDATE.
This year was just the syph test - no xrays, but i was weighed and measured.
I am shorter this year - crumbling under the weight of nonsense heaped upon my weary shoulder.
Had to wait a bit longer - after syringing my arm of blood for the syph test, the nurse discovered she had run out of sample bottles to put the blood in.
Somchais mo cy taxi company managed to source said sample bottles, probably off the local market.
^ "How bout I don't have sex with the children I teach and you skip the syphilis test" is too logical for Thais.
I was aware of the requirements but I actually didn't need an x-ray or have to test my urine. I went to Bumrungrad, paid 600 Baht. Had my height and weight checked, blood pressure etc and had a doctor check my heart, lungs and eyes ears. A cursory stomach poke and I was done. Just tell the hospital what you need the exam for, they have a form ready to go.
Never had a problem...
I renewed mine last week. I was in Phuket on holiday and went to Dr Bangla which is right on Bangla Rd. Nurse measured BP while waiting for the Doc. Cute lady Dr, listened to my chest and back, palmed my stomach (I had serious stomach surgery a couple of years ago) and that was it. Certificate in Thai, 300thb and I was away. Left it with HR in BKK on my way back and the new permit arrived by EMS on Saturday morning.
No blood test, no X-rays.
I gotta renew mine next week at one stop.
no medical required.
So, can I ask... do you have to have a business in Thailand to give yourself a work permit? And, to have your own business there, do you need to own a property to register it as the HQ? You have to have pretend Thai staff? I'm just curious.

You don't "give yourself" a work permit. You apply for one from the local employment/labour office.
