If a human has a uteris and 2 x chromosomes it's a female, if it has testicles and an x and a y chromosome it's a male, same as it ever was regardless of the fashion of the times.
Having said that every culture has and has had its transgenders, there's always a percentage that bend that way, from Thailands ladyboys to Samoas Faʻafafine. Funnily enough neither culture has had an issue with it. I don't know if they get the knickers in a twist over BS like 'preferred pronouns' though.
