...indeed, some pearled members are more of a challenge than others...but the Isaan members (a friend has seen) usually display just a spherule or two...not a challenge to an experienced pearl diver...

In the Pacific, although not common, it does occur. Not done as babies but done by the men themselves. If you catch a big fish (100 kg ish), the eye is boiled and the centre of the eye is a chalky sphere. Remove the chalky substance to reveal a hard pearl-like ball (the same goes for smaller fish like you may buy whole at a restaurant, ...but the inner pearl is smaller than birdshot).

An incision is made to the skin of the penis shaft and the pearl inserted and the wound left to heal. Some guys will have a row of them.

Tell your friend that I imagine a bit of scar tissue on the top of one's shaft will not affect one's own pleasure significantly. If it does reduce sensitivity it may even prolong performance.
Just my guess.
