Being fat is the result of bad health choices, unhealthy eating habits or metabolic disorders. Human society has never been perfect. Being fat/unhealthy is just icing on the cake. There is a cocktail of other social problems that are far more severe, needing serious quick answers.

Its a tricky situation because I believe in promoting self confidence and being proud about one's self, but parading a very fat person promotes unhealthy eating habits, so does parading stick-like figures.
I suppose acceptance, tolerance and discretion must come into play.

Hey, if they are happy then I'm happy for them. If the girl is happy with who she is then kudos to her. Women can still be curvy and healthy.
People can choose who they want to be with, keep the judgements to yourself imo.
I've learned that people who judge other people and the way they look aren't happy themselves.

I dress to impress myself first and also to fit into the dress codes at work. When I'm not at work i'm mostly dressed down into comfortable clothes. I noticed in different cultures people dress differently to impress.. for example, in Korea many women and men dress smartly all the time. You see them on the subway dressed to kill. In that society, people are very surface and face is important as you say. Cosmetic surgery is also huge in Korea.
I don't find the people dressed as smartly in China, but you see them wearing a lot of knock off brand name clothes.
In Canada, people don't dress overly smart unless they work in a place that requires them to do so. I find it is also very individual and people all dress differently from dollar store clothing to the brand names.

There needs to be more healthy models, and I'm not talking about obese, but people that have curves and are healthy (size 10 12, 14).
