Never had one made in Thailand but I had one made by the dentist in the uk. Can't remember how much it cost. Probably not much.

Well, to get a tooth implant with all the trimmings at a hospital such as Bumrungrad, it costs ~ 120K baht.
Check in any Muay Thai training studio and I believe there are a few off Sukhumvit.

When I was a kid playing American football you just would goto the local sporting goods store and buy one. When you got home you throw it in a pot of boiling water and then put it in your mouth and bite down. Instant custom mold. I am sure that it is still done that way.
If it's for sports, getting one custom-made really isn't necessary- the ones made by Shock Doctor are great and you can fit them yourself- I've used a few different ones, and I've found the 'Gel Nano' to be the best (and Amazon will ship it to Thailand cheaply).
