None of the above ! And I'am sure that in Pattaya he would have gotten the same treatment.
Why ? Because every hospital in Thailand smells big money when german tourists need treatment. They just think that every german is insured.
I remember once that a person in white (I'am not going to call them doctor) told me: "Why are you worried about the cost ? You are insured ! "

Kudos to the hospital for caring for him.

I intially agreed that it was a fine gesture from the hospital but later suspected that the injury may have not been the German's fault and the Airport Authority footed the bill due to negligence on their part.

The hospital had to have got the money from somewhere and I feel it was from the Thai Government offices and until they pulled the plug.
