No helmet, thank God cos I hit the corner of the kerb with my eyes. If I'd worn a helmet the visor would have killed me. I was very, very lucky.
My best friend got wiped out by a hoon in a ute a few years back. Pete was fully protected with everything money could buy. Hoon's doing 90-100, Pete.... maybe 50. Dead as.
All the helmets in the world won't count more than sensible parental care and protection.

I am not aware of any suitable motor bike helmets for sale anywhere in the world and for youngsters just say 2-5 years of age.
Lets face it you don't see many people in western countries driving around with their wife and small child as pillion passengers on motor bikes.
Maddies suggestion about producing a special lightweight helmet for children has a lot of merit after not only seeing what I see here but also in Vietnam. I would imagine the harness mechanism would have to be right but the idea is certainly worth some thought.

Really to make a lightweight high impact (polycarbonate/ ABS mix) shell with a special adjustable harness is a fooking great idea and the asians will buy it for their kids even if they don't wear one.
Get governement safety approval and a bit of funding covering the local's purchase and you are on your way mate.
