I went to visit a company logging camp in Tele, Indonesia one time and was given the mini tour including the vehicle workshop where they changed the skidder tires. The guy, a Kiwi, was telling me how hard it was to get local mechanics to use the cage when inflating tires for the first time and proudly showed me the hole in the workshop roof where one guy went through. Said he left the hole there as a reminder for the rest of the guys!
