Four die in Milford Haven Refinery Explosion

Apparently during routine maintenance of a storage tank. I'm guessing a not empty storage tank.

HOUSTON -(Dow Jones)- Four people died and one was hospitalized with serious injuries after a storage tank exploded Thursday at Chevron Corp.'s (CVX) Pembroke refinery in Wales.
The explosion occurred at 18:22 local time in Milford Haven as workers were performing maintenance, according to the local fire department. Witnesses cited by the BBC said there was a "blast large enough to shake the windows."
The company will continue to investigate the cause of the accident at the 220,000 barrel-a-day refinery, refinery manager Greg Hanggi said.
"This news is utterly devastating," Hanggi said. "The loss of our co-workers has come as a huge shock to us all."
Local fire fighters extinguished the blaze quickly, according to the Mid and West Wales Fire and Rescue Service. The explosion, fire and resulting smoke should have no effect on the local communities near the refinery, Chris Davies, the service's assistant chief fire officer, said.
"The wind was blowing off shore, away from residential areas," Davies said. " Members of the public can be reassured that there is no ongoing risk to health as a result of the incident."
Chevron has agreed to sell the Pembroke refinery to Valero Energy Corp. (VLO) for $730 million, plus an additional payment estimated to be $1 billion for inventory and other items. The sale is expected to close in the third quarter of this year
