Fear of what, you idiot? It's on your back, so whatever they will be doing, you won't see it.

Thai hospitals and doctors are very good at pain control, and chances are the are will be so numbed you won't even know they are doing anything at all. When it's done, you'll have a bandage and instructions to change it daily with some antibiotic cream. As long as you follow the instructions, there'll be no pain -- the longer you wait, the bigger the complications when you finally do something, and the bigger the likelyhood of pain.
