The discussion on various threads is always lively, especially concerning what one office wants versus another as far as documentation.
Being curious (which I've read is mostly fatal only to cats) I want to know if; When you do your yearly extensions of stay do they use any of these forms?
I also thought it would be worth my effort to be a "kinder gentler Tod Daniels" in the interest of fostering glasnost between posters in different regions of the country
In Bangkok you submit all of these for a yearly extension of stay based on 'retirement'.
This one is just titled "Statement" (หมายเลข 3) basically is a self guarantee form (it is inane in its repetitiveness and you hafta write your name, address phone number on it in at least 3 different places!)
This one is "Acknowledgement of Terms and Conditions for Permit of Temporary stay in the Kingdom of Thailand" (แบบ สตม. 2); basically is says you're gonna follow the rules concerning your permission to stay and your stay could be revoked if you change the reason of your extension or violate the terms.
This one has no name but says you know the financial requirements for extensions of stay based on retirement going the banked money route is 800K for 3 months prior to applying. The totally strange thing is; I tried leaving it out of the documentation with someone using an embassy letter for proof of income, but they handed it to me and made the person sign it anyway.
This one is that new "Acknowledgement of Penalties for Visa Overstay" (even though these new overstay rules are NOT yet in effect, because they're gonna do a cabinet vote to pass them).
This one is not being handed out when you get extensions of stay, BUT instead they are being handed out to ANYONE applying for a re-entry permit. It's called "Notification Form for House-Master, Owner or the Possessor of the Residence Where Alien has Stayed" (ตม. ๓๐). It's where a thai declares there's a foreigner living at the place they rent.
Now we as foreigners renting shouldn't even be required to do this one. In fact the b/s printed information card stapled to it which they hand out about it with engish on one side and thai on the other totally says it's for the thai we rent froms job to do this. When I asked the immigrations staff why we as foreigners were filling it out they said, "No one ever did it so we started making you guys do it.." Go figure.
This was "one rope I didn't try to push" with them. It's an easily downloaded and filled out form and while it's not our responsibility to fill it out, I know how to "pick my battles" with immigrations, this was not one.
BTW: once you fill this one out you get a receipt showing you've already turned in one of these. DON'T take that form outta your passport OR you'll hafta fill out the form again, EVEN though the data is in their computer already.

"Phuketrichard", "cnx37" or anyone else. How are the forms up/down in your neck 'o the woods? Same, different, never heard of 'em. I'd like to know, just so we all can know..
Oh one other thing. IF any of the immigrations offices you go to uses these forms (especially the ones you can't download), AND you want good quality pdf's of them. Send me a P/M and I'll get it to you. The better prepared you are goin' to where ever it is you go, the faster you can get outta there.
