It is OK, full of overblown expats and people there recommended by their hotels, but top end prices
a nice glass of house wine is B250+
nice to just sit outside in the yard, in a tented seating area
depends on what type of atmosphere you like
as said before, Marcos is much more relaxed and the food more innovative, the prices better; a glass of house wine around B150 gives some idea relatively

If you want a nice place away from the mayhem, check out the Al Dente. Best lasagna in CM and it is in a very secluded area. Heading toward Doi Suthep on Huay Gaew Road, take the first left after the Canal Road intersection. This soi is about 10 feet past the Dunking Donuts next to the Phucome Hotel and hard to find. After you turn left, follow the signs with the big tooth. Al Dente has great service and a Thai/Italian menu. Crispy Italian pizza and definitely off the lazy farang/tourist route.
If you like Mexican, Salsa Kitchen not far from Central on Huay Gaew Road is great. (Park at the shell Station) It's up there in our top three cafes and they serve a fusion style of Mexican and we always get filled up there. Lots of water throwing out front.

Oldish thread, but with a few suggestions that haven't been made here yet.
I always liked Buenoissimo for Italian. Last time I was in CM it was on the riverside, up the road from the Hollanda Montri guesthouse.

Marinakorn - good value buffet, farang and thai though thai is better
and Horizon Garden Resort - farang thai and suchi buffet ( garden visit included with free golf buggy tour)

Anyone who describes the Dukes as a "typical shit US-style fast food dump" has obviously never eaten there. It is a bit pricey but the food there is excellent, great pizza cooked in a wood fired oven and mouth watering fare on a good menu.
My favorite place for Indian food at a very reasonable price is a little place in the same soi as the Number One bar, off Loi Kroh Road. Just go past the bar and it's about 50 meters on the left. They are very nice people and they make beautiful curry dishes, lamb and beef. The owner also sells food in the Anusan Market at night in the food area on the left as you enter the market.
Dalaabaa is a lovely restaurant, very pretty, good food, acceptable prices, located at 113 Bumrungraj Road.
They have a website under their name, check it out on the internet.
Tha Naam on the river, about 3 - 5 Km south of the night Bizarre/just before Mahidol rd; Thai fare, reasonable prices, river front dining no fucking tourist crowds. relaxed dining. Hanglae is excellent, the Kantoke dinner is really a platter of appetizers small serving of Hanglae, fried fermented pork ribs, nam sod, naam prik ong, etc. - get that, some rice (sticky or steamed if you must) and an Entrée. Wine prices? Thai food is best served with an icy cold lager, sadly Thailand has shit beers. They do have a couple boats for diner cruises. One is perfect for a single party of two, You are far enough off the tourist path to make most of the experience a treat. Stay in and be serenaded by soft voice of a Thai singing on key.
Houn Suntree; again on the river. Slightly busier than Tha Naam, good quality Thai food, The owner is well respected entertainer and she is usually on hand to sing. Occasionally her daughter or other folks will sit in, Soft jazz guitars and some Thai/western hits.
Both places offer good service and excellent Thai food. Houn Suntree can get busy at times and Song Kran is one of those times. You won't be pressed in like sardines as with those places along the Farang stroll(riverside, good view, etc.). if I had to pick one it would be Tha Naam.
An adventure to get to is Tharntong Lodge & Restaurant. It's about an hour out of CM City (maybe Amphur Mae On?) directions... Uh... Call them. (53-275125) An excellent place to get out of the SK madness. They are in hill country. They have Bungalows/rooms. The food is excellent the service is unobtrusive yet prompt. There is a wine list but I've never sampled any from it. Sit out over the stream that meanders through the grounds and feast on Goong Den, Nam Si Klong Moo, and several other northern delicacies. This is a place to relax, and if you happen to looking for "Romantic" this is it. Rooms come with an "American Breakfast" that is anything but. No matter. Hot dogs, boiled ham and over-cooked eggs are a Thai delicacy and you should know that by now. Opt for the Jok; savory and delicious.
If you are more than five years past puberty and do not consider a five day, non-stop alcohol-fueled water fight the height of entertainment This may be the best option on the menu. "The Flight of the Gibbon" is about 10 Km further up the road, if you're into that sort of thing. Home stays are available, the lodge enjoys a proximity to nature and the grounds are pleasant enough to wander around, just don't step on the Rabbits, chickens or let the Sulphur Crested Cockatoo intimidate you (he's friendly and speaks a smattering of Thai).
Re: Dukes. Analogous to U.S. working-class luxury, 'cept the food isn't quite as good. The food isn't bad, for the price it's a very good imitation of western chain dining like "Claim Jumper" or "Hungry Hunter." I like some of the appetizers, the ribs are pre boiled, rendering them somewhat tasteless. The steaks are OK, you can do better at the supermarket. Beer and wine is served but nothing to get excited about. The service is up to the aforementioned working-class luxury standard. Nearly eight years in CM and I've been there maybe six or seven times. First time I went I was not disappointed my expectations were toned down after a few years living in Thailand.
Miguels was mentioned and if you're into Mexican it is good, the service is at the counter and wine list consists of Tiger, Sing, Heineken and soda pop. Huevos Rancheros is great, the Taquitos are nice, the rest is so-so in my opinion. Menu was designed by a New Jersey native transplanted to California for a while, he is also the guy that built the menu at Mike's Burgers. Mike's has gone down hill and if you need a burger go to The Chiang Mai Saloon. There's a burger joint across the street (Chang Moi?) from Mikes that was doing a much better burger - one problem except their sliders all burgers are half pound monstrosities. ANY of the joints mentioned in this paragraph are in the thick of things, the food would probably be the height of fine dining if you are of the mind-set that would enjoy the moat area or Loi Kroh during Song Kran. The roads outside and inside the moat are grid-locked with drunken fools in pickups sloshing water at the pedestrians sloshing water, There are enterprising young ladies selling Canned beer from truck to car to motorcycle. the moat is probably being cleaned right now in anticipation of all the folks that will draw on that for their ammunition, the feeder roads are littered with equally drunken and obnoxious revelers. I think the TharnTong Lodge, or something similar your best bet. Out of the city center with things to do that don't involve going into town or dealing with anyone out side of your maid and meal servers. I can't say enough good things about the place. the rooms looked nice but I only viewed one and did not stay. An added bonus; the place is at about 2200 ft above sea level, well forested and much cooler than in town. That can be important mid-April.
Caveat: I've been away from CM for about 4 months now. Things might have changed drastically. But I doubt it.

Never too late mate, I arrived yesterday at 3 pm and spent a fun 3 hours driving from the airport to the hotel on Charoen Muang Rd - it's the Shewe Wana Suite Resort which I would highly recommend. So far we've munched on the street and at the hotel but will be out tonight to take in the Riverside area. 3 more days after that so will look into one or two other suggestions for a scoff...
Songkran is indeed a fine slice of lunacy up here...

For "After SK" dining my fav. Italian in Chiang Mai is still the Arcobaleno...
